Sign Up Now Before It’s Too Late. ” Actually, on April 4, 2014, the morning of my surgery, it was already four days too late to sign up for insurance under the Affordable Care Act, or Obamacare. Besides, I already had decent insurance. But at least that dream was more on point with what was happening in my real life. The day I found out about the time bomb in my chest, I was finishing reporting for a book about Obamacare and the fight over how to fix America’s healthcare system. In fact, on March 31, 2014, the day I was told about my aneurysm, I was awaiting the results of the final push by the Obama administration to get people to enroll in the insurance exchanges established under Obamacare.

To the extent anyone could figure it out, the First Lady’s plan seemed likely to force most Americans to give up the insurance they had in favor of these new government-supervised co-ops. The key industries—insurers, drug companies, hospitals, doctors—had ended up opposing it. The drug companies famously sponsored a series of ads featuring a middle-class couple, “Harry and Louise,” whose plain talk about keeping the government out of “our healthcare” galvanized the public, most of whom had private insurance and feared radical change.

There were multiple analogies that people used for the entrenched, sorry state of the American healthcare system. One man’s toothpaste tube was another man’s tangle of legacy programs and delivery systems that, piece by piece, had been patched together into an unworkable mess that provided substandard care at exorbitant prices. Trying to tack new programs and systems onto that was a recipe for frustration. It was, to use yet another metaphor, like deciding to fix up a hopelessly dilapidated house, instead of saving time and money by building a new one, because so many people had so much invested in preserving the irredeemably defective plumbing, wiring, and floors in the old house.
